Hair Loss Treatments – A Doctors Eye View

A short while back the German Society of Dermatologists took a long hard look at hair restoration. We take a look.

 

Hair Restoration

In a report that was eventually published in 2011 the German Society of Dermatologists set out to achieve something not seen before. Their noble ambition was to provide the medical profession with some solid information on which to base their advice to hair loss sufferers, specifically sufferers of androgenetic alopecia, that inherited form which affects over half of all adults and is currently incurable.

Methodology

Using a set of criteria they examined the pros and cons of all the principal hair loss treatments on the market. So with separate consideration for each treatment in regard to men and women, and based on a range of attributes, for example the ability to prevent progression was considered along with safety, the quality of the evidence available and the improvement offered.

Results

The notable high score for hair restoration came in the quality of the evidence available. Because it is an invasive surgery the work is performed by a qualified, and heavily regulated, surgeon. With everything reported there is plenty of data on which to draw meaningful conclusions. As might be expected it scored a zero for it's ability to halt progression of the hair loss but shaded both minoxidil and finasteride for it's ability to improve the appearance. One last observation was that in terms of safety a transplant was considered less safe than finasteride, which in turn is more risky than minoxidil.

It's Different For Girls

There was a note of caution for women in the report. For women the ability to improve appearance was only rated as high as the only FDA approved medication. This is due to the nature of hair loss for women, who tend to see their hair thin across the top of the head. Men will usually lose hair at their hairline which slowly creeps backwards. It is an easier task for the surgeon to replace that hairline than it is for him to fill in thinning hair.

At last! A serious study looking at a topical version of the hair loss cure hopeful, JAK inhibitors. We look closer.

 

Aclaris Therapeutics

We have been watching their progress for what seems like forever, such is the glacial pace of progress in the world of new medicines. Aclaris Therapeutics are another of the new breed of biotech companies looking to leverage their ever expanding knowledge to deliver new medications, in the case of Aclaris the focus is on treatments for skin and hair conditions... where there are currently no FDA approved medications or where gaps in treatment exist. Stands to reason then that hair loss, particularly the form labelled androgenetic alopecia, would be in their cross-hairs.

JAK Inhibitors

The Janus Kinane pathway (JAK) is thought to be responsible for delivering messages to the follicle which cause it to stop in the rest phase, which occurs just after a follicle has shed it's hair. After briefly staying in that rest phase it should move in to the anagen phase, where active growth occurs. JAK inhibitors work by interfering with those signals and allowing the follicle to move through that rest phase and into anagen... it certainly seems to have done the trick for a number of mice in the early trials of Aclaris' snappily named ATI-502.

The Trial

Aclaris have announced a Phase 2 open-label study of their topical JAK inhibitor, ATI-502. It will involve a dozen men and women, so 24 in total, being treated twice a day over 30 weeks at three different facilities, all in the USA. In the words of D. Stuart Shanler, Chief Scientific Officer at Aclaris, "This is an important step forward in understanding he clinical utility of our JAK inhibitors in patents with androgenetic alopecia."

Exciting news from the USA on a subject we were only moaning about last week, the lack of a study on JAK inhibitors seemed strange given the encouraging results for the oral version. Any oral medication will take years to come to market due to the stringent testing and barriers to market entry. A topical version bypasses many of these hurdles and can be monetized quickly as an over the counter product, so long as the trials do not show any adverse effects... which has been something of a problem for the oral version. 2018 Catapult Summit to Accelerate Children’s Cancer Cures

Alopecia Sufferer Wins Little Miss Wisconsin

Joslyn Larson is not your typical beauty pageant winner. The second grader from Eau Claire in Wisconsin has set an example to young girls everywhere that beauty is at least as much on the inside. Joslyn was diagnosed with alopecia areata,  two years ago. A condition that often presents as coin-sized discs, though can be more severe and certainly was in Joslyn's case. Since then she has undergone numerous treatments but to no avail. Eventually her parents took the decision to discontinue attempts to halt the condition, on the understandable basis that they did not want to subject her to the steroids and other medications when they were proving of no value. Headbands and hats were Joslyn's solution. Right up until the day she came home from school and announced that she was shaving her head. "My hair was falling out anyways and I thought it was really annoying me so I shaved everything off."  Her supportive mum said "The best choice we had was to make sure she was confident, and comfortable and happy. That's the best medicine we could give her."

Little Miss Wisconsin

Joslyn's can do attitude extended to entering beauty pageants, which her mum described as a positive experience building young Joslyn's confidence and letting her meet new people. That confidence clearly made its mark on the  judges at Wisconsin's premier beauty pageant for young girls, the Little Miss Wisconsin event. Joslyn walked off with the big prize and is now looking forward to representing her state at national level when she attends the final in Orlando, Florida later this year. Her proud parents were of course delighted, in the words of her father, Jake Larson "I'm not much of a crier but I teared up a little bit last night when I saw her get crowned and the sash and all of that and yes, I'm just a proud daddy."

Ruxotilinib Making Hair Loss News

Ruxolitinub is suddenly in the news, approved in 2011 it has been touted as a possible hair loss cure. We take a closer look.

 

Ruxolitinub

When it was first approved for use in medical treatment it was as a treatment for a particular form of cancer.  Myelofibrosis, a rare bone marrow cancer, had been shown to respond positively to what is known in the medical world as JAK inhibition. So, ruxolitinub works by interfering with the pathways that signal the autoimmune response, by inhibiting the JAK pathways along which the signals for that autoimmune response travel. Occasionally these responses can go into overdrive and cause problems. In the case of alopecia areata, that form of hair loss which sees the victim develop coin sized patches of baldness, the immune system is tricked into believing that non-threats like the hair follicles are actually a threat and trigger the target and destroy reaction. It was not necessary to be an eagle eyed research student to notice that some of the patients being treated for bone marrow cancer were unexpectedly regrowing hair... nor did it take a Nobel prize winning scientist to realise that the action of inhibiting those JAK pathways was the cause.

Results

Studies were conducted on the efficacy of ruxolitinub to treat a specific form of alopecia, alopecia areata. This condition is directly associated with an autoimmune dysfunction and so clearly had potential to be positively treated. In one of the larger studies 12 subjects with severe alopecia areata were given a 20mg dose twice a day for between three and six months. The results were highly encouraging though with some side effects. To date we are only aware of two very small studies for a topical version of the treatment. A teenage girl who had a bare scalp and underarms as well as a sparse right eyebrow. Her eyebrow regrew and she saw 10% growth on her scalp. The only other study we know about was not so encouraging, a 66 year old saw no growth at all.

Despite the sudden burst of articles, including one in the UK's best selling daily tabloid The Sun (see their article here), there is no immediate prospect of a topical version, and an oral version aimed at treating hair loss will take much longer to go through the process of FDA approval. Nevertheless, JAK Inhibitors are the focus of research for labs around the world and we watch their progress closely. Fingers crossed. Hair Loss During Chemotherapy

The often dramatic hair loss that accompanies chemotherapy treatment is usually the first visible sign of the sickness. The emotional effects on the patient can be dramatic, indeed for some the prospect is too terrible to consider and the offer of therapy is declined. It has also been cited that some 8% of women who undergo chemotherapy would, in hindsight, have preferred to keep their hair. There is no cure for chemo induced hair loss although some patients will regrow hair naturally in time. Endocrine Therapy

Usually in the form of tamoxifen or aromatase inhibitor, endocrine therapies have made a huge difference in cancer care - In the words of Mario Lacouture MD, senior author on the study, "There are tens of thousands of breast cancer survivors on endocrine therapies, and the remarkable improvements in patient survival that have been achieved through their care have led to patients now being concerned about quality of life issues, like partial hair loss." His team looked back at 112 women who had endocrine therapy and who had no history of alopecia (nor any other scalp condition). While they found that 100% of the patients developed hair loss only 8% of those developed what his team refer to as grade 2 alopecia.. it is not the hair loss scale we are familiar with but basically means that you are classified grade 2 if you lose more than 50% of your hair. Dr Lacouture's team noted, unsurprisingly maybe, that the negative psychological effects of hair loss were present even if the hair loss was grade 1.

The good news for endocrine patients is that 80% of them treated with topical minoxidil showed improvement... though their study did not look at the effect of starting minoxidil prior to treatment. Those 8% of women who, having undergone treatment and lost their hair, would turn back time and avoid the treatment are a clear sign that this is a real problem and worthy of attention. At just 18 months old, Addison was diagnosed with Stage 5 bilateral Wilms tumor. Hope was just over the horizon for Addison as the cancer ultimately went into remission – but not before a long and tumultuous treatment process.

When Addison was diagnosed – she was too young to comprehend what was happening to her. After a routine check-up, her doctor sent her for an ultrasound which ultimately identified the tumor on her kidney. Just one day later, Addison would have a port placed and started chemotherapy just five days after her diagnosis. She completed six months of weekly chemotherapy sessions, some lasting hours at a time and she had 11 consecutive days of radiation to both her abdomen and chest to help address areas not removed by surgery. She also had a right nephrectomy three months into treatment to remove her right kidney and the entire tumor that encompassed it.

“Addison’s treatment affected us in many ways, but at the time, we were all in. We had family support, and support from our workplaces and friends to get through those months,” said Jaime Barber, Addison’s mom.

Addison also took part in a clinical trial and in October of 2017, five and a half years after diagnosis, she completed her final scans and was given the all clear – the most precious words a family can hear about their child battling cancer. A heady mix of social media and celebrities are being used to shill hair loss supplements of extremely dubious value, and some threat. We look closer.

 

The Supplement Industry

It was an industry born in that most optimistic of decades. In the 1960's Linus Pauling, at one time hailed as America's greatest young scientist, received a letter. It came from a man, who it turned out had spent just two years studying chemistry at college, suggesting to Pauling that if he took vitamin C every day it would add 25 years to his life, maybe more. At the age of 65 you can see why Pauling was willing to forgo his usual scientific rigour and give it a go. So convinced was Pauling of the positive effects of his daily dose of vitamin C that would become responsible, in 1970, for a book titled "Vitamin C and the Common Cold" - in it he claimed that it was possible to eradicate the common cold. It became a best-seller and set the scene for the enormous global supplement market we see today, projected to become worth $220 billion by 2023. Ironically, at least 15 major studies since Pauling published his book have not only not been able to support his theory with evidence, they have actually

A Bizarre Blend

More recently we have seen the advent of supplements aimed at hair. Their spectacular growth has been entirely down to their modern marketing techniques which see celebrities like the ubiquitos Kim Kardashian, who started out promoting "Hairfinity vitamins" before trading to support Sugarbear Hair gummies, marketed as candy that saves your hair. While Kardashian was criticized for failing to tell that she was paid by the company those little chewy sweets now have 2 million followers. That is despite Buzzfeed reporting that independent analysis of Sugarbears found that the ingredient amounts differed by up to 20% to what was on the label, as well as 70% more biotin... compare the recommended daily amount of 30 to 100 micrograms to the 5,000 contained in the sweets and ponder on just how pointless that is... especially as biotin deficiency is actually very rare.  Oh, and they also found small amounts of lead. Lead. In a candy. Quite takes your breath away doesn't it? Owners of other hair supplement manufacturers tell that they deliberately produce packaging and product designed to be visually shareable. Indeed, one large brand raised $5M of funding with one investor crediting their strength on social media as a major factor.

Cooling Cap Hair Loss Saver Hits Snag

Cooling caps have made a mark on the world of chemotherapy, but the latest news suggests they might struggle to achieve widespread availability. We look closer.

 

Cooling Caps

The idea was a simple one,  that by significantly cooling the scalp, thus reducing blood supply to the area, fewer of the toxic chemicals used during chemotherapy will reach the hair follicles. The chemicals used in chemotherapy take deliberate aim at fast growing cells, of which the cancer cells are some. Unfortunately so are those involved in hair growth which is the reason a completely bald head on a young person is often the only outward sign that they are ill. Losing your hair at a time when you are already going through the trauma of coping with cancer can be a daunting prospect... one which prevents some women from even accepting the treatment - however crazy that might sound to some. Cooling caps were shown to have a positive effect, though far from a guarantee against hair loss they were the first product to market which even tried to address the issue. They can, and do, make a profound difference for some patients and for certain types of chemotherapy they have achieved great results. We have, over the last 18 months or so, seen their use proliferate across continents as they gained approval by the various national agencies that control the introduction of such equipment... including the USA.

A Price On Your Hair

A problem has arisen around paying for the cap. With a typical cost of around $2000 for a bespoke fitted cap most would need their insurance to cover that expense. Unfortunately the insurance companies are baulking at the cost with many suggesting the results are not definitive enough to support their effectiveness. The two small scale studies that have been done suggest otherwise, that these caps definitely improve outcomes. Hospitals that offer the caps will do what they can to help patients using discretionary funds, though these are usually earmarked for transport or medications so there is doubtless a fight being had for those resources.

Childhood Cancer Survivor to Spend 12th Birthday Volunteering at 28-Mile Charity Hike

Eleven-year-old Will Busby first heard of CureSearch’s Ultimate Hike seven years ago, when his father, Jeff, starting raising funds and training for a 28.3-mile trek to be completed in one day. The family was well aware of the need for funding for children’s cancer research, as four-year-old Will had been diagnosed with high risk acute lymphoblastic leukemia on in 2011.

“More funding for research is critical. One in eight children diagnosed with cancer won’t survive, and of those who do, 60% will develop lifelong side effects,” said Brecka Putnam, senior manager of campaign development. “Only four treatments have been developed specifically for children in the past twenty years. We need more treatments for children to improve patient outcomes.”

Will Busby

The Busby’s have been actively involved in research since his diagnosis. Will participated in a CureSearch funded clinical to help further research, and Jeff laced up his boots and raised funds through the Ultimate Hike program. Jeff continued hiking and coaching new hikers, and Will continued fighting cancer for years finally the family celebrated his “no more chemo day” on May 6, 2014.

Although Will is officially done fighting cancer, he’s chosen to spend his 12th birthday volunteering on the hike to show his support for all the kids who are in treatment now. “He now seems to be dealing with the experience in a way that makes it a positive part of his story,” said Jeff Busby.

Volunteering for an Ultimate Hike is no small feat. Jeff will spend the day on the trail, coaching other hikers and setting them up for success. Meanwhile, Will’s day will be spent loading and unloading vans, filling up hikers’ water packs, cleaning, and cheering on hikers. The first volunteers will report for duty before 2:00 a.m., and the last volunteers on the trail could load up as late as 9:00 p.m.

“We’re so excited to have Will spend the day with us. It can be exhausting, and many kids would prefer to spend their birthday hanging out with friends, rather than making sandwiches for 70 people in the middle of the woods. We’re so thankful,” said Putnam. “Having someone that young who beat cancer out there on the trail will give our hikers the motivation they need when the going gets tough.”

In addition to volunteering with CureSearch, Jeff is a chaplain and leader for Will’s trail life troop, he also backpacks with the group once a month, all year round. He is looking forward to introducing Will to a new group of hikers in a few weeks.

Jasmine Collins has established her salon, Razor Chic in Atlanta Georgia, as a go to for African American women suffering with traction alopecia. This form of alopecia, for which there is no cure, is typically bought on by styles that see the hair pulled into tight braids, or the wearing of heavy weaves and wigs which put pressure on the natural hair they are attached to. The problem is not restricted to African American women of course, but it is undoubtedly more of a problem among them due in part to style preferences. The problem is exacerbated because African American hair, due to it's curly nature... it means the sebum, hair's natural protector, cannot travel easily round the bends in the hair making them more prone to breakage than straight hair. This form of alopecia will typically appear at the hairline where the pull on the hair is at its greatest, but it can result in more significant and widespread hair loss and leave the sufferer with some tough choices. Siegel first became interested in the area of pediatric cancer asa resident at the University of Minnesota. This was during the 1960s, when mortality rates for pediatric cancer were extremely high; he saw that the field was overwhelmed with countless unanswered questions and he was driven to participate in finding solutions. His commitment dates back to a time when industry leaders like him were just starting to understand how to treat pediatric cancers. In just one example, Siegel was among those who pioneered the use of sterile environments for patients getting intense chemo or bone marrow transplants.

From developing the first pediatric protective environment in 1971 for children undergoing intensive chemotherapy to pioneering academic and clinical care programs nationally and locally for adolescents and young adults with cancer, Siegel’s contributions have revolutionized the field of pediatric oncology. He has been the recipient of numerous awards from major organizations around the world for his efforts to end children’s cancer. Although his list of accomplishments is vast, he’s not done effectuating change.

“Most people are aware of the fact that we’ve had tremendous improvement of survival rates for childhood cancer, however, those strides come at a price,” said Siegel. “Two-thirds of those patients have one or more side effects as a result of their treatment and at least half of these can be life-threatening side effects. We need to find a way to treat them without the side effects or with less of an impact on their lives and long-term health.”

Siegel has played an integral role in the implementation of CureSearch’s Catapult Strategy. This game-changing strategy identifies issues in the pediatric research space and has identified ways to overcome them. With the help of thought leadership, CureSearch is changing status quo to advance the strongest children’s cancer research so treatments are widely available for the children who need it most. Siegel explained that the Catapult Strategy is crucial in bringing all the stakeholders together to make this possible.

He also noted that by convening philanthropists, academia, industry, individuals and other stakeholders in a coordinated effort, CureSearch leads in catapulting new treatments – a major focus of the 2018 Catapult Summit. This annual conference brings together industry leaders who are committed to accelerating the development of pediatric cancer treatments.

That has changed dramatically and I felt we really needed to try to stimulate the industry and get over these barriers to develop drugs for pediatric cancer. Siegel is currently the Chairman of the Board for the Health Care Foundation for Ventura County. He is also a member of the Global Board of Ronald McDonald House Charities. He has been involved with numerous philanthropic efforts across the country to support pediatric cancer research and has published over 250+ peer review publications in the areas of pediatric cancer and blood diseases. He has been heavily involved in international health programs and helped develop the Institute for Pediatric Oncology at Moscow.

Is FMT Really The Answer To Hair Loss?

We wrote recently about FMT and it's extraordinary curative abilities, including to restore hair. We look closer.

 

Faecal Microbiota Transplant (FMT)

As we reported previously, see the first article by clicking here, FMT was reported in 2013 in the New England Journal of Medicine. It told the story of a research study which had seen the team involved take stools, that is poop to you and me, from carefully screened donors. This poop was then processed to extract the healthy bacteria which lives, or should live, in all our stomachs... all of us carry around between 1 and 2Kg of bacteria in what medical people refer to as our gut microbiome. The theory was that the balance of bacteria in their patients could be improved by providing healthy bacteria from other people.

Result!

The results of that initial study were beyond any reasonable expectations, they stopped the study on the grounds that it was considered unethical to withhold the treatment from those on the study receiving the placebo. Everyone on the study was cured. It was the first indication that the gut microbiome was a much more significant player in our general well-being than had ever been appreciated.

New Studies

A new trial at the Massachusetts General Hospital has been producing faecal capsules (it seems the T for transplant in FMT is a mild over-exaggeration these days) from lean people and giving them to obese ones. In 2015 it seems a woman cured of C. difficile using the treatment suddenly developed obesity, from bacteria provided by her obese daughter. In Australia, Professor Thomas Borody, at the Centre for Digestive Diseases, produced a study showing that 18 children with autism, who suffer with bowel problems, treated with FMT not only had their gut conditions improved, the classic autistic issues of poor social skills and communications improved too. There are also clinic trials exploring it's potential in treating multiple sclerosis and it is touted as a possible weapon in the fight against the modern plague of hypertension.

The fact that it cured two people of all over hair loss has implications for it's use to regulate problems with the autoimmune system, which is at the heart of hair loss. So while Professor Borody warns "... the trouble is that we could have the following: people wanting faecal transplants so they can be taller, or to make their hair grow." New Hope

We are spoiled for choice these days when it comes to smart research teasing us with ever more in depth understandings of the processes around hair growth and loss. New gene therapies create exciting storylines on an almost weekly basis. But news of an original, to put it mildly, method of recalibrating our body's systems reaches us that makes for exciting reading. One which has been shown to improve the condition of people suffering from anything from obesity to autism... and, of course, our pet favourite, hair loss.

Snake Oil

Since records began we can see men worrying about their hair loss and taking extreme measures to attempt a save. Ancient scrolls and papyruses have contained unusual suggestions from pigeon poop to cow saliva as possible cures. To date though, nothing really works - with all the benefits of modern science we still only have two FDA approved medications, one topical and one systemic (which is not available to women who represent approximately half of all hair loss sufferers). Snake oil salesman reached their peak in the 19th and early 20th centuries. When people's faith in science to change their world was understandably high, at the same time as their grasp of science was still rooted in centuries old misunderstandings. It was a relatively easy task for a salesman to pass through a town and spin a convincing enough yarn to peddle their wares.

Modern Snake Oil?

It is difficult to describe this new treatment for long to someone before their eyebrows begin to knit together, puzzlement quickly gives way to incredulity and you will find yourself having to reassure them that what you are saying is not only based in fact but appears to be a genuinely original area of medicine. One that is not based on the medicines we have developed over the last century and a bit, and not one that is based on any of the new gene science. This is something altogether different.

Faecal Microbiota Transplant (FMT)

You may want to let your lunch go down before reading on... but make sure you do read on - this is amazing stuff. Back in 2013 the vaunted New England Journal of Medicine published a study of FMT. The study centred around a type of gut inflammation triggered by the notorious bug C. difficile... this is a disease that kills one in ten of it's victims. FMT is a process where donated stools, taken from carefully screened donors, are processed to remove the healthy gut bacteria, which was then, back in 2013, placed inside the gut of the patient (the treatment is now available in capsule form). It is possible the reason for the study being stopped has happened before, but we have not heard of it. They stopped this one because it was ruled unethical to withhold the treatment from the study participants who were not receiving the full treatment because it had cured everyone who did. Phenomenal.

For Hair Loss

A couple of unexpected outcomes from FMT treatments have indicated that our gut bacteria may play an important and influential role when it comes to our autoimmune system. One of these saw US-based gastroenterologist, Colleen Kelly, publish details of two cases where patients suffering from an extreme form of alopecia that saw them lose all body hair made a full recovery.. both from their case of C. difficile and from their hair loss.
