What Are The Limits Of Hair Transplantation?

A recent article praising the quality of a celebrity hair transplant stopped us in our tracks, we take a closer look at just what is realistically achievable for a hair transplant… and what is not.

 

Antonio Conte

As a player, Antonio Conte was a legend. Playing at Juventus, Italy’s senior club, he won 5 titles, and every cup tournament possible, including the Champions League, club football’s most competitive competition. 

As a manager, Conte set out on his career in the Italian lower leagues. He clearly bought the same qualities he showed as a player to his new role and won promotion as well as manager of the year. That talent would bring him to the attention of the club he had represented so successfully as a player and, as manager of Juventus, he led them to three consecutive Italian titles. From there he would be tempted to Chelsea in the English Premier League, the world’s most competitive. He repeated his success and took the title in his first season followed by the FA Cup, the sport’s oldest cup competition, a year later. Earlier this year he returned to Italy as manager of Inter Milan.

Hair Loss

As a professional footballer, Conte had to endure the pain of hair loss at an early age in the full gaze of his adoring public. Once his career was finished he took the perfectly understandable decision, like so many others, to do something about it. No doubt he spent his time searching for hair growth solutions that actually work before deciding on something more permanent… and quicker to achieve an end result. Essentially, to get to where is today his choice boiled down to one between a hair transplantation procedure and a hair system of one sort or another.

Limited Options

For us, his choice was, very clearly, to go for the system. While a top surgeon, with the right tools and canvas to work with, can produce a convincing hairline (witness James Nesbitt), what still remains beyond them is to take a Norwood 5/6 client and provide him with the tumbling, full fringe we see on Antonio Conte. His search for a hair loss solution that actually works will have told him that there is plenty which can be done. Though when Conte was doing his search, around the year 2000, some of the more modern treatments were not yet known to science. 

Ultimately, that choice would have boiled down, given his clear desire to return to a full head of hair and to turn the clock back over a decade, to a straight one between transplantation and a system.  But any honest surgeon would have given him a clear picture of the maximum achievable, and it would have been substantially less impressive than the youthful mop he now sports.

Say What You See

When we picked up an article proclaiming the quality of Antonio Conte’s hair transplant we were taken aback, how could anyone with any experience of hair loss seriously think this was the work of a surgeon? The answer was nobody. In fact, the article turned out to be a well presented but shameless advert for the work of an Indian hair transplant business.

There is some wonderful work being done by hair transplant surgeons. For the right client with realistic ambitions, the results can be wonderful. But articles like the one proclaiming Conte as one of those clients only serve to undermine that good work. Anyone considering surgery would be well advised to perform due diligence. Elevated Sunburn Danger For Bald Spots

As everyone begins to enjoy the long hot days of summer we bring a stark warning on the importance of paying attention to UV protection. This is true for everyone, but particularly for men in the throes of hair loss for whom a bald patch brings its own set of additional dangers. We look closer. 

 

UV Protection 

We are all aware, to some extent, of the dangers of UV rays from the sun. If you live in a hot climate you will have seen how those powerful rays, from all of 93 million miles away, are capable of destroying all but the most ruggedized of outdoor furniture. A typically bright fabric deckchair in the south of Spain will do well to last into a second year – the colour will be bleached before the stitching slowly disintegrates and the whole thing falls to bits. Those invisible rays are brutal. 

Those same rays are capable of devastating human skin. Severe cases of sunburn are linked to triggering skin cancer. Witness the Australian cricket team taking the field, even when they tour England, coated in Zinc Oxide… not for them a Factor 50 cream, they go for a solution that allows zero UV rays to reach their skin. 

Bald Spot Sunburn

So why the warning on bald spots? There is, of course, the rather obvious starting point that the suns’ rays land directly on top of our heads when we are moving around outdoors. Without the proven protection, trusted for millennia, of hair to protect the skin a bald spot become the easiest place to get burnt. 

So we wear hats or slap on Factor 50, or Zinc if you are Australian. But remove that hat for just 15 minutes and your bald patch can burn. A bald head sweats on hot days, quickly removing any cream and requiring much more regular application than most realize.  Over time, continued exposure to the sun will cause the skin on the scalp will get thinner with immune and regenerative properties diminished. 

Slow Healing

Beyond the increased dangers there is also another downside to having a bald spot. Once burned, the skin on a bald scalp can take much longer to heal. Elsewhere on the body, the skin is regenerated using stem cells. But on the scalp, those stem cells are found in hair follicles. So if you think your sunburned scalp takes a long time to heal, its because it does.

UK Team Work To Improve Hair Loss Cooling Caps

A team from Huddersfield in the UK are working hard to achieve their aim of zero hair loss from chemotherapy. The latest news out of their lab on their work with hair loss cooling caps is encouraging. We look closer.

 

Cooling Caps

These wonderful devices burst onto the scene relatively recently and for the first time offered patients undergoing chemotherapy some hope of holding onto their hair. The problem is with the way the chemicals in chemo work. By setting out to target fast-growing cells like the ones in the tumour they end up taking out other fast-growing cells in the body. Hair is right up there and almost invariably an early casualty of the treatment. 

Cooling caps work by, as the name suggests, cooling the scalp of the wearer prior to treatment, during treatment, and for a while after. The theory being that the reduced blood supply to the region sees less of the harsh chemicals getting to and killing the hair follicle. American studies showed a 50% success rate for hair retention for the current generation of the cap, but a team in Huddersfield have been beavering away at improving on that and claim to be on target to get it up to 80% by as soon as next year.

Key Improvements

Scientists at the University of Huddersfield in the UK have been working on a two-pronged attack to improve hair loss outcomes for chemo patients. The first involves producing a bespoke 3D printed cap for every patient. The idea is that the snug fit would improve the effectiveness and efficacy of the treatment by making perfect contact with the entire hair growing region of the scalp. 

The second element of their plans involves an ointment. To be applied a couple of hours before treatment and possibly again just after… they are still working on the fine detail.  The idea is to further prevent those chemicals from finding their way into the hair follicle and enhance the work done by the cooling cap itself.

Castor Oil Making A Hair Loss Comeback

Castor oil has been recognised for its beneficial properties for a very long time. Modern appreciation of its complex structure has only enhanced its reputation, particularly as a hair loss salve. We look closer.

 

Castor Oil

In our childhood, castor oil occupied a unique place. Understood to be ever so good for you, a spoonful could be offered either as medication or as punishment, such was its reputation for having a terrible taste. 

Its ancient use as a laxative was described in a 3,500-year-old papyrus, by when it had already been in use for centuries.

Our ancestors were right to recognise the potency of castor oil. Using modern techniques we have a much better grasp of just what this versatile vegetable oil is capable of. That versatility sees it used for a breathtakingly wide range of purposes: soap; lubricant; brake fluid, paint, dye, coatings, ink, cold resistant plastics, polish, nylon, pharmaceuticals, and perfumes. More recently, castor oil has begun to appear in all manner of beauty treatments, including some aimed directly at hair loss.

Does It Work?

There are a few good reasons why castor oil has become the new go-to ingredient for hair loss sufferers. It is extremely high in vitamin E and a monounsaturated fat called ricinoleic acid. 

Ricinoleic acid is important, it acts as a humectant… which means it prevents moisture being lost through the skin. It also has anti-inflammatory properties. The combination means that when applied directly to the skin it can help balance PH levels and help prevent dandruff or flaky skin. In addition, it can boost circulation to the scalp… a vital element of healthy hair growth.

A Father’s Story

Travis Stafford is a father and childhood cancer advocate. He is committed to finding new treatments and cures by supporting CureSearch programs. In his mind, his son George’s future depends on it.

George, Wilms TumorTravis Stafford’s perception of fatherhood changed after his son George was diagnosed with Wilms tumor at age 7. “The dad is supposed to be the rock, protector and provider for the family. I came to realize that is was okay to let my guard down and just be—be there, be scared and be emotional.”

In July 2014, just after his 7th birthday, George complained of a stomachache on and off for a couple of days. Travis and his wife Jen took him to the pediatrician. “It didn’t take long to know that something was terribly wrong.”

As George laid down on the examination table, they could see something six inches long protruding along his right side. He was rushed to the hospital immediately. “All they could tell us at the hospital was that it wasn’t leukemia and we needed to get to Primary Children’s Hospital in Salt Lake City (350 miles away) as fast as we could.”

“At first, you just go numb and try to take in all of the information.”
“That week was really a blur-tests, doctors, nurses, techs, social services. It was a lot to take in. After his first MRI, we heard those damn words, your child has cancer.” George was diagnosed with anaplastic Wilms tumor (stage 3), the most common form of kidney cancer in children.

Before he could have surgery, George received two months of chemotherapy to try to shrink the football-sized tumor that had taken over his left kidney. Surgery was followed by more chemotherapy alternated with radiation treatments at University of Utah’s Huntsman Cancer Institute. Straddling long stays in distant hospitals and maintaining family life takes a lot of support from others. “Swallowing my pride and accepting outside help was the hardest thing. I wanted to be in control, and I couldn’t. Cancer was.”

George & Travis

Thankfully, after a year of treatment, George was sent home cancer-free in August 2015. Looking back, Travis shared some advice for other cancer fathers, “Cancer moms develop an unbreakable bond with each other. Dads need that, too. It’s okay to cry, to be scared. Find cancer dad groups locally or on social media. Talk about it. Share your feelings, your anger and your knowledge. I feel like dads are the forgotten group. People always ask, ‘How’s your wife, your kids?’ Not many people ask how the dads are doing.”

“No kid should have to go through this, period.”
“No one cares about childhood cancer until their child or grandchild gets cancer. It’s time that changed! George is not a cancer survivor; he is surviving cancer. The same treatments that cured him can still give him cancer-maybe not today or tomorrow, but someday. He has had a few scares, but it does not hold him back or keep him down. That’s why it’s so important to raise money and awareness for children’s cancer research.”

At Primary Children’s Hospital, the Stafford’s received CureSearch’s educational resource materials for families. Later on, they learned about CureSearch Walks and CureSearch’s mission to fund critical research to find new, less-toxic treatments for children’s cancer. Now, the family is organizing their own DIY Gold Fundraiser for CureSearch. The inaugural George Says Cancer Sucks Bowling Tournament is coming up on August 24, 2019 in St. George, Utah. “This is our first, of hopefully many, bowling events to raise money. We need an army to fight this!”

China’s Booming Hair Transplant Market

China is witnessing exponential demand for hair transplant operations, what is driving this trend and what are the implications for the rest of us? We take a closer look.

 

Chinese Hair Loss 

We recently covered news out of China which looked at the results of a huge survey on hair loss. The work was carried out by the China Association of Health Promotion and Education (CAHPE) and it concluded that China had 250 million hair loss sufferers, of whom 160 million were men. The numbers are enormous, but then so is China and statistically, they might be about what was expected. But another survey, carried out on 2017 on 4000 students at Tsinghua University, discovered that an amazing 60% reported thinning hair, and 40% reported a receding hairline. 

Hair Loss Solutions

The result of an economy that has experienced double-digit growth for decades has been an explosion in the size of the Chinese middle class, an affluent sector with a very Western attitude to consumerism. Taobao, the Chinese E-Commerce giant, reported that there were several hair loss products among their 40 best selling lines. The list includes everything from minoxidil to vibrating hairbrushes and adhesive hair pieces. What was truly remarkable about the Taobao results was that they showed 40% of the sales of these items were made to customers under 30 years old.

It seems a reasonable assumption then that the problem of hair loss for younger people is real, and that the desire to do something about it is facilitated by the availability of western treatments and tools. 

Hair Transplantation

With hair loss becoming a major concern for these young affluent workers it is probably no surprise that an industry has grown to meet their needs. In 2017 iResearch reported that the hair transplant market in China was already worth $1.37B… but it was their claim that growth has been in excess of 100% year ar on year. 

Liu Zheng is CEO of Lotus, one of the major chains of private clinics with 30 branches across the country. He told China Daily that he expects the exponential growth to continue and predicts a market worth over $10B within a decade. He points to the development of techniques and equipment in China that will continue to improve outcomes and drive down costs. So that despite the possibility of a general slowing down of the country’s growth rate the health of the hair transplant industry looks secure.

UCLA Scientists Access Unique Funding For Hair Loss Cure

Scientists at UCLA, a leading American university, have accessed a unique funding vehicle to further their research into an exciting possible hair loss cure. We look closer.

One of America’s top twenty universities, the University of California, or UCLA to its friends, is listed in the top ten for its Public Health department. Great news for us since it is a group of scientists from that elevated team who have announced results of some interesting research into hair loss. 

Leads on this research were three faculty members; Heather Christofk (Ass Professor of biological chemistry; William Lowry (professor of molecular, cellular and developmental biology; and Michael Jung (distinguished professor of chemistry and biochemistry). The three have set up a company with the aim of focussing on hair loss.

Research 

The team was looking at processes involved in the growth of the hair follicle, particularly at the stage where it enters the actual growth phase to produce new hair. 

They first noticed that the metabolism of the hair follicle was completely different from that of other skin cells. Cellular metabolism breaks down the nutrients needed for cells to divide, for energy, and to allow them to respond to their environment. Hair follicles take glucose directly from the bloodstream which they process to produce a metabolite called pyruvate which it sends to the mitochondria where it can be used to create energy. But it also converts some of that pyruvate into another metabolite called lactate. The team developed a theory that if they could reduce the entry of the pyruvate into the mitochondria they could force the follicle into producing more lactate… which might activate the cells and see hair grow more quickly. 

To test their theory they first genetically modified mice to block lactate production and showed that this stopped hair follicle stem cell activation. Then, working in collaboration with a team from the Rutter lab at the University of Iowa, they showed that by increasing the supply of lactate they accelerated that activation.

They went on to identify two drugs that, when applied to the skin, were capable of affecting the rate of lactate production.

Funding

So far so good. But the costs associated with taking an interesting drug through to trial is prohibitive and many interesting research projects disappear at this stage for that reason. 

Fortunately for the team at UCLA, the university has an Innovation Fund. Seeded with cash from the California State it exists to help research teams develop ideas into products, to turn ideas from the biomedical teams into treatments that can benefit the public, and do it quickly.

The fact that this one comes from UCLA adds gravity, and their funding facility should be applauded… something we might all be grateful for down the line if their treatment comes to market.
