A threat to hair has been identified after a French doctor warned of a "toxic association" between hair loss and a popular plant group. We look closer.

 

Time For Tea

There were two cases that triggered a closer investigation by Philippe Assouly, a French dermatologist, into the properties of the humble pumpkin. A woman in France had eaten a bitter tasting pumpkin soup. Her family, who had been with her for the meal, had eaten less of the soup and all suffered food poisoning. The woman in question experienced nausea, vomiting and diarrhoea in the immediate aftermath. A week later she began to lose hair from both her scalp and pubic area. As reported by Assouly in the Journal of the American Medical Association, a second woman suffered with food poisoning after eating a meal that included squash, another member of the same family, when other diners had avoided the squash due to it's bitter flavour. "About three weeks later, the patient experienced substantial hair loss on her head as well as severe alopecia on the armpits and pubic region." Assouly said.

Root Cause

The plant family in the spotlight are cucurbits, it includes pumpkins, squashes and gourds. They can all contain cucurbitacin, a toxic compound which produces a bitter taste. Typically that bitterness will have been bred out of the domesticated cucurbits grown for the food industry. Insects can carry the pollen from wild plants and cross pollenate them with the domesticated versions, thus reintroducing the potential threat back into the food chain.

The bitter taste is there as a warning and should save anyone from eating too much of a bad thing. Popular Hair Loss Drug Gets All Clear For Cancer Risk

One of the two FDA approved hair loss drugs has been exonerated, in newly published research, of a link to prostrate cancer. We look closer.

 

Finasteride

Marketed primarily under the name Propecia, Finasteride is one of only two hair loss drugs that have been ratified as safe to use by the US FDA. The story, completed by this latest publication of results, has been 25 years in the making and has had its ups and downs in that time.

Prostate Cancer Prevention Trial (PCPT)

The principal investigator, Dr Ian Thompson (pictured), has overseen this remarkable study which began in 1993. Between '93 and '97 nearly 19,000 men took part in a randomized trial. In 2003 they announced that the medication reduced the risk of prostrate cancer by almost 25%. That was a huge outcome but it was tempered by some results, from the same study, that indicated a slight increase in the number of high-grade prostrate cancers. A negative finding that resulted in warning labels being added to bottles.

Eureka Alert!

This month at the world's largest gathering of urologists in San Francisco, Dr Thompson will present the findings of their 25 year study on the effects of finasteride. He will confirm to the audience that the final results show there is no statistical link between the medication and any increase in the incidents of prostrate cancer of any kind. In his own, understandably excited, words: "What we can now say is that finasteride not only significantly reduces a man's risk of prostrate cancer, it is safe to use based on very long-term follow-up in our study. In PCPT we found no increased risk of prostate cancer death in men who took finasteride compared to men who didn't. These results are transformational. Prostate cancer is the most common cancer diagnosed in American men and we have found an inexpensive, effective drug that can prevent it. I'm pleased to report that we've answered the questions and closed the book."

Jada recently explained that the reason she has been pictured wearing so many turbans lately is due to hair loss. She had been talking on her Facebook show, Red Table Talk, to her daughter and her mum and broke the news, not to mention a taboo... it is highly unusual for Hollywood A-Listers to discuss hair loss, almost unheard of for the person doing the talking to be a woman. It begins, for many, by noticing more hair in the plughole. At least that is the case when the hair begins at the crown of the head, where it most unlikely to be spotted by the wearer and much more noticeable to those around them. If they're lucky the comment will come from a sympathetic corner, a family member or close friend. If not it can happen at work or socialising, we have read so many of their testimonies among the tens of thousands of posts to our forum and these harsh moments of discovery clearly leave their mark.

What Should You Expect?

Genetics play the primary role but individual hair loss is just that, individual. While there are a few broad truths, that the hair loss will almost invariably start at the temples or crown for example. But hair can be lost in either location and not in the other. You may see your temples recede with no significant loss at the crown, or the other way round. While it is more common to have both it is far from standard. The other main variable is the pace of progress. At one end of the spectrum we hear from men who lost their hair in a matter of a few months, sometimes while still in their teens. When we say lost their hair we mean they went from a full hairline to a Norwood Scale 7 where just an arc of hair remains at the back of the head, connecting the ears.  The good news is that not all will progress to that extreme stage. Nor is it a linear path to the end of your hair loss advance. Some will see hair recede suddenly then stop, for no obvious reason. It might continue again a few months later, or stay roughly where it is for years... just one of the unknowns confronting a transplant surgeon when considering where to locate those follicles.

Splitting Hairs

Your hair basically comes in two types. The hair from the back of your head is different, it is why transplant surgeons harvest the follicles to be moved from that region. Those follicles are genetically programmed to be stem cells, so they do not shed. Just how many of those type of follicles you are born with will dictate the ultimate extent of your hair loss.

A fair portion of the globe tuned in to watch the marriage of Prince Harry, sixth or so in line to the British throne and younger brother of Prince William who will inherit the crown on the passing of his father... though his father has been kept waiting for his turn by the Queen who is now in her nineties. Prince Harry has been an extremely popular member of the royal family, he was always  on solid ground with the public who watched him as a very small boy be made to walk behind the coffin of his mother. He grew up, somehow, with a touch of the everyman about him, incredibly difficult to achieve for a royal - the evidence of his uncles shows just how hard. Uncle Edward always managed to look like a very posh fish out of water in just about any situation. Uncle Andrew has spent the last few decades swanning around the world like an old fashioned playboy. Harry on the other hand spent his time in the army and came out a passionate supporter of his injured comrades working tirelessly to raise their profile and cash for their support.

A Very New Sort Of Princess

The high regard he was held in by the public was only enhanced when he announced his engagement. There is no doubt the Brits love a royal wedding, but this one was destined to be special from the outset. The Prince was to marry an American divorcee, with echoes of an ancestor who was forced to abdicate the throne when he made the same choice. Not just any American divorcee either, this one was a TV star with mixed racial heritage. UK Woman Credits Shampoo As Her Alopecia Cure

Last week in the UK the Daily Mail bought us the story of a UK woman in her forties who found her own alopecia miracle cure on the supermarket shelf. We look closer.

 

Belinda's Story

Belinda Clarke, from Swindon in England's South West, is 44 years old and has suffered almost her whole life from stress associated with her alopecia. From the age of 4, when a traumatic event triggered the condition for the first time, Belinda has had to cope with bald patches that progressively got worse until she became almost completely bald. Her difficulties began when she started primary school where she was bullied and called "baldy." A tough start for a young girl and Belinda took to wearing a wig or putting in clips to cover her patches. As she grew up with the condition she took to wearing a baseball cap if she had to venture out, but rarely beyond the local shops. A wig would give her enough confidence to attend family events, but over time these became her only excursions.

In her own words; "I started using on my bald head in September 2016 and within only three to four weeks I actually noticed tiny hair growth all over my head. After a week which saw two genuinely new avenues of research open up for hair loss researchers we look at yet another possibly promising development...

 

New Hope

The story we covered earlier in the week around the team from the University of Manchester, they had discovered a wonderful side effect of the treatment for osteoporosis they had been studying, it seems to work at the molecular level to block a powerful brake on hair growth. A long way from a product but always exciting to read about something genuinely new. Within a couple of days we were reading about another team on the other side of the world, in Japan to be precise, who have made some ground breaking discoveries around our gut bacteria and the influence that might have on healthy hair growth... exciting because we have seen other stories of remarkably beneficial discoveries involving gut bacteria which seems to play a far more central role on our broader well-being than previously believed.

Three In A Week

In an unprecedented flurry of original news stories around hair loss cures we nearly fell off our chairs when we found a third in less than a week.  While this one is possibly not on the same level of research as the two earlier examples we liked it because it targeted women in particular. An article in the International Journal of Dermatology described a trial on 100 sufferers of female pattern baldness using 0.25mg of minoxidil, the FDA approved hair loss medication, and 25mg of spironolactone, a medication used as a treatment for fluid retention in people with congestive heart problems or cirrhosis of the liver and to prevent low potassium levels.  Both minoxidil, the only FDA approved hair loss treatment for women, and spironolactone have separately shown that they can slow down or stop hair loss in women, and even regrow hair in some cases, the study was conducted to see whether together they could provide greater benefits.

Results

Dr Rod Sinclair, the study author told Healthline (see their article here) "The combination of spironactolone together with minoxidil was tolerated well by almost all women, arrested hair loss and actually regrew hair in most women." Phase II trials have already begun with Dr Rodman predicting a 6 month wait before they will be ready to go for status as a pre-IND (Investigational New Drug) meeting to map out the path for evaluation and development of this treatment so that it can be made available to women affected by hair loss in the US and worldwide."

In the modern world of social media how else does a troubled superstar ask for help with a personal problem... Sarah Hyland chose Instagram.

Known by millions as the eldest daughter in the hit comedy show "Modern Family" Sarah Hyland has suffered from birth with a challenging condition called kidney dysplasia. Sufferers from dysplasia have one or more kidney where the internal structures of the organ do not develop properly. So instead of regular kidney tissue they develop fluid filled sacs, cysts, which can cause a range of reactions from pain and high blood pressure to urinary tract problems and the risk of damage to the healthy kidney where only one is affected. When Sarah was diagnosed at the age of 9 she already had an enviable list of great shows on her acting CV. She described feeling tired and sick if she did not get 12 hours sleep. The result was that she would try and recover between takes, as she described it "You know, if you're sick you still go to work, and in between takes you sit down, or you lay your head down or something." Six years ago Sarah received a kidney from her actor father giving her a new lease of life, something an estimated 4,500 others will die waiting for. She continues to work as an ambassador with the Lopez  Foundation, set up by actor George Lopez to raise awareness about kidney disease and organ donation.

Hair Loss

Now 27 years old Sarah has now taken to Instagram to try and find the solution to a new problem, her hair loss. Pointing at the medication she continues to require Sarah attempted to crowd source some advice from her legion of followers. "Help, help, help." She wrote "What hair-care products are out there for thinning hair? This 27 year old would like to know" She went on "I know it seems like I don't but I do. Some of my medications have been making my hair fall out." It seems that some of the more popular products are unavailable to Sarah because of the medications she continues to require so she was looking for alternatives. Her 5 million followers did not let her down either, she was inundated with ideas. In the end she posted to say she had settled on a mix of castor and coconut oil and avocado mix, "maybe I will put some vitamin E in there. I don't know" she added. News from Japan details some fascinating new research into the role our gut bacteria might play in hair loss. We look closer.

 

Gut Bacteria

The role that our gut bacteria plays in our broader health is only recently beginning to be understood... or at least grasped. The link between the immune system and the bacteria was highlighted by research. Mice born with without any bacteria lack important immune cells, but add even a restricted amount of bacteria and the immune system develops and produces diverse cells. That research has led to a good understanding of the effect of individual bacteria, as well as groups of bacteria, on the function of the immune system.

Bacteria and Hair Loss

Earlier research had shown that bacteria free mice that lack biotin in their diet will suffer from mild hair loss... pointing to the possibility of a bacterial imbalance being the cause. A team of scientists in Japan set out to learn more. They gave mice a diet with and without biotin and saw no hair loss. When they repeated the experiment at the same time as giving the mice a long course of antibiotics aimed at destroying the balance of bacteria in their gut. As expected the mice suffered mild hair loss. By watching exactly what happened in the gut bacteria during this process the scientists noted that a bacteria called Lactobacillus murinus had expanded during treatment. It could not produce biotin and so was identified as the likely cause of the biotin deficiency that triggered the hair loss. Indeed, when they fed bacteria free mice with the same bacteria they saw their mice become almost completely bald. In a final test they gave the mice the bacteria but a diet with regular levels of biotin.. and saw no hair loss. They also found that they could prevent the hair loss with injections of biotin.

What Does It All Mean?

Understanding how bacteria influences hair loss will offer new opportunities for treatment. The teasing possiblity is a simple probiotic supplement which would inhibit the biotin-eating bacteria that the Japanese team has shown plays a role in hair loss.

It seems probable that rather than any one eureka moment the cure for hair loss will be an amalgum of several strands of research and results. He tells us "I'm no expert about what women want in a man, but I'm sure that a good 90% of them don't care less how much hair you have, as long as you're not an entitled idiot and respect them as people..."

Vice is a website and magazine that carries professionally written articles alongside personal contributions and provides an eclectic spread with something for everyone. Our eye was well and truly caught by one of those personal contributions from Julian Morgans. The article tells us it was first carried in the Australian version so our assumption is that Julian is an Aussie. But wherever he is from we suspect that his story will strike a chord with many of our readers.

Early Hair Loss

Like many sufferers Julian first noticed he was losing his hair while still in high school. To be specific, like many sufferers it was someone else who drew his attention to it... "Man, what's up with your hair? You've got a bald spot." Julian's first hint of his troubles to come was a bolt out of the blue, someone standing over him on a sports field had spotted what Julian had failed to... his receding hair. Julian goes on to describe the classic symptoms of a young man trying and failing to come to terms with his condition. How he would "tune out" of conversations and find himself inspecting his friends hairlines. We particularly liked his description of developing "bat-like awareness of anyone approaching from behind or standing above."  The hours spent in his parents bathroom using a hand mirror to inspect the top of his head. So, describing himself as obsessed and angry he tried every medication he could find regardless of cost.

Flirting With Finasteride

Julian found himself in a clinic being quoted $4000 for an annual supply of medication. He was 17. Such was his obsession with his hair loss that. He deferred university so he could work to earn the money to pay for his medications. Eventually he would settle on a blend of the two FDA approved medications, finasteride and minoxidil. Now 31 years old Julian has dropped the minoxidil but continues with the finasteride... despite some fairly horrendous sounding side effects he attributes to the drug. He says his eyes are yellow and doctors have failed to diagnose the mystery pain in his groin, though it flares up 20 minutes after taking the finasteride and lasts a few hours before fading. So although he tells us the doctors suspect the finasteride he says "... the choice is that or the baldness, so I keep eating the pills. And actually, I'm one of the lucky ones." He goes on to describe the symptoms of "post-finasteride syndrome" which we will write more about in an upcoming article. Even though the symptoms of this syndrome have been hotly disputed Merck, the patent holders, have settled with hundreds of plaintiffs in a case which came to a conclusion earlier this month.

It is impossible to read Julian's article without feeling enormous sympathy. At the same time the frustration of realising this is an intelligent young man who has already passed up on significant life opportunities by allowing his hair loss to hold him back. Even now we see him committed to a medication regime which he knows to carry risk and which seem to be manifesting for him... yet he sticks with the finasteride.
